(Step 1) Membership Application
Our general membership application gathers the necessary information we need for your child to attend the Boys & Girls Club of Moorpark & Simi Valley, it also gathers information needed for grant funding purposes. (Please complete all sections of the Membership Application) If you are a current 2026 Member, you do not need to fill out the Membership Application.
(Step 2) Parent Agreement Form
The parent agreement form provides families with the Summer policies and procedures for the Boys & Girls Club of Moorpark & Simi Valley.
