We are Closed on July 3rd.
(Step 1) Membership Application
Our general membership application gathers the necessary information we need for your child to attend the Boys & Girls Club of Moorpark & Simi Valley, it also gathers information needed for grant funding purposes. (Please complete all sections of the Membership Application) If you are a current 2026 Member, you do not need to fill out the Membership Application.
(Step 2) Parent Agreement Form
The parent agreement form is to make sure all families understand the policies and procedures of the Boys & Girls Club of Moorpark & Simi Valley.
